I'm not sure how old this booklet is, but there's a very good chance that it has been greatly improved and enlarged over the years with further research. Caleb Johnson's Mayflower website is one of the best: MayflowerHistory.com

Mayflower passenger John Cooke, who came to Plymouth as a 14 year old boy with his father Francis Cooke, settled in what's now Fairhaven about 1662. His homestead was less than half a mile down Main Street from where I'm sitting as I type this. When Cooke died here in 1695, he was the last surviving male passenger of the Mayflower.
